Martin Virtual School

MVS Full-Time K-12 Registration for the 2009-2010 School Year



To be eligible for the program, students must meet one of the following:
  • Has spent the prior school year in attendance at a Florida public school.
  • Is the dependent of a member of the military who was transferred in the last 12 months (proof of transfer required.)
  • Was enrolled during prior school year in K-8 Virtual Program under s. 1002.45, F.S.
  • Has a sibling in participating K-8 virtual program that attended during the prior year.

Martin County School District will notify families of acceptance (or denial) into MVS no later than August 12, 2009. If accepted, families will be directed to the online registration with Florida Virtual School Connections Academy/Florida Virtual School which must also be completed.  Instructions will be provide when notified of acceptance.

Registration requirements are defined below for:

Current Martin County School District Students must submit the following:
  1. Student Registration Form
  2. Records Request Form
  3. MVS Program Information / Participant Agreement Form
*** Families registering more than one child must submit one for each child.

Original Documents must be submitted by 5:00 p.m. on September 4, 2009 to:

Enrolling New to Martin County School Students must submit the following:

  1. Student Registration Form
  2. Records Request Form
  3. MVS Program Information / Participant Agreement Form
  4. Proof of residency (purchase contract, or lease/rental agreement or current FPL or cable statement)
  5. Proof of physical examination within the last 12 months – Form 3040
  6. Proof of Immunization on Department of Health - Form 680
  7. Original Birth Certificate
  8. Copy of the last report card

*** Families registering more than one child must submit one for each child.
