2008-2009 Immunization Information

Any child entering a Florida school for the first time shall present, or have on file, a Certification of Immunization on Form 680.  For incoming students, fully immunized means the following: 

Kindergarten through 7th Grade:

  • Five doses of Diphtheria, Tetanus, Pertussis Vaccine
  • Four doses of Polio Vaccine
  • Two doses of Measles, Mumps, Rubella Vaccine
  • Three doses of Hepatitis B Vaccine
  • Varicella Vaccine:
    • Kindergarten students are required to have two doses of the Varicella Vaccine (if the student has not had the Chicken Pox). 
    • Students in grades 1 through 7 must have one dose of the Varicella Vaccine (if they have not had the Chicken Pox.)

Students in grades 7 through 12 are also required to have:

  • Tetanus (Td) Booster or Tetanus Accellular Pertussis Booster (DTaP)
  • Hepatitis B and MMR vaccines must be up-to-date

Students who are not fully immunized will not be allowed to start school.

 

All children entering the Martin County School District for the first time must also present a physical exam (on form 3040) that was completed within a year of school entry date and signed by a licensed physician.
